Welcome to the Parent Portal

Please read all instructions before proceeding to the online forms:

- All UIL forms, PPE’s, AISD Emergency Contact, Consents, and Release forms are required to be completed annually for all incoming 7th-12th graders.

- We recommend that all parents create an account. Without a parent account you will not be able to see your student's status or download the completed forms. By creating an account, it also allows us to send notes and messages regarding your student when necessary.

- Once a parent account is created, search for your student by Last name and/or Student ID

- Please double check that all sections of the physical form have been completed, and both student and parent/guardian signatures are on the bottom of the front Medical History page in preparation for uploading.

- Begin clicking through each of the links of the various online forms. Sign each one electronically and save to move on to the next form.

- Upload a PDF version of your completed pre-participation physical (both front and back pages) when prompted. By uploading an electronic copy, you will not need to submit a hard copy to your coach or Athletic Trainer.

- Once you have completed all electronic forms, you will have the opportunity to print all documents.

- Pre-Participation Physical Forms are available at the following locations: Anna ISD Athletic Office, Anna ISD Athletic Training Rooms, using the “Download and Print” tab, or downloading it from the UIL website.
